Apple Watch: What the Research Says About Each Metric
Apple tracks more individual metrics than most competitors, but combines none of them into a single daily score. Here's how each measurement actually holds up.
Apple's approach is breadth without synthesis. It reports eight distinct health metrics, each with its own accuracy record, and does not combine HRV, sleep, and heart rate into one composite number the way Oura's Readiness Score or Whoop's Recovery Score do. That's a real design choice, not an oversight, and it shapes what this page can and can't tell you. Below is what independent research and Apple's own technical documentation say about each metric it tracks, plus two things it deliberately doesn't.
How Does Apple Watch Measure HRV?
Apple Watch reports HRV as RMSSD, measured via wrist PPG, mainly surfaced through the Breath app or an Apple Health data export. It doesn't display a running HRV Balance or trend score the way Oura and Garmin do.

"Heart Rate Variability" in the Health app. Apple doesn't expose the raw HRV time series directly to developers, which limits how third-party apps can build on it.
Worth knowing
Apple's RMSSD usage rests on secondary-source agreement rather than a primary Apple statement, unlike its VO2 max methodology, which Apple has published directly.
How Does Apple Watch Measure Resting Heart Rate?
Apple takes random samples whenever its accelerometer detects stillness throughout the day, then averages them, an approach that can exclude overnight data entirely.
What Apple calls it
"Resting Heart Rate," same name every other brand uses, but the measurement window is different: Garmin uses the lowest 30-minute average in 24 hours, Oura and Whoop lean on overnight lows. Apple's daytime-sampling approach means its number can genuinely differ from a ring or a Garmin watch worn by the same person on the same day.
How Accurate Is Apple Watch's VO2 Max Estimate?
Apple calls this "Cardio Fitness," not VO2 max, and reports it as one of four tiers (Low, Below Average, Above Average, High) rather than a raw number by default. It's estimated from outdoor walking and running heart-rate response, not a graded exercise test.

Apple deliberately renamed the metric to "Cardio Fitness" instead of using the scientific term, which means someone comparing an Apple number against a Garmin VO2 Max reading may not realize the two devices are estimating the same underlying thing.
Worth knowing
This is one of the few Apple metrics with a genuine primary source: Apple has published its own technical methodology paper on the walking/running heart-rate approach behind the estimate.
What Does Apple Watch's ECG Actually Detect?
Two separate features get conflated constantly: the Irregular Rhythm Notification is passive, PPG-based background monitoring, while the ECG app is a genuine single-channel electrocardiogram, FDA-cleared, that requires actively holding the digital crown. They don't carry the same evidence quality.

How Does Apple Watch's Blood Oxygen Feature Work?
The Blood Oxygen app takes an on-demand spot-check reading lasting roughly 15 seconds, available on Series 6 and later plus Ultra models. As of this writing, it is not continuous or automatic overnight in the US market, a real difference from Garmin's and Oura's overnight tracking.

How Does Apple Watch Track Activity and Calories?
Step count comes from a 3-axis accelerometer inferring wrist-movement patterns, not literal footfall detection, and accuracy drops sharply at slow walking speeds or when wrist movement is restricted, a limitation shared with most wrist-worn wearables.
Worth knowing
A systematic review of 65 studies across 72 devices and 29 brands found energy-expenditure error exceeded 30% for every brand tested, including Apple, even though step count and heart rate performed comparably better in the same review. Calorie tracking is the weak point across this entire product category, not something specific to Apple.
What Apple Watch Doesn't Do
Two real, deliberate absences worth knowing before comparing Apple to a device built around them.
No native recovery or readiness score
Whoop's Recovery Score, Oura's Readiness Score, and Garmin's Body Battery all combine HRV, resting heart rate, and sleep into one composite number. Apple reports the individual measurements and leaves synthesis to the reader or a third-party app.
No native stress score
Apple Watch collects HRV, heart rate, and motion data, the same underlying signals Garmin's Stress Score and Oura's Daytime Stress are built from, but doesn't publish a stress metric of its own.
If independent accuracy for each individual metric matters more to you than a single daily composite number, Apple's breadth-without-synthesis approach is a real fit. If checking one score each morning matters more, that's not what this device gives you natively, Whoop, Oura, and Garmin were each built around exactly that instead.
