Apple Watch HRV Accuracy Against ECG and Chest Straps
Question assessedHow accurate is Apple Watch heart rate variability (HRV) compared with an ECG or a medical-grade chest strap?
Less closely than its heart rate. In 39 adults, Apple Watch Series 9 and Ultra 2 read HRV 8.31 ms lower than a Polar H10 chest strap on average, with a mean absolute percentage error of 28.88 percent, outside a 10 ms equivalence margin, while resting heart rate was accurate. In 78 adults, a Series 6 matched ECG beat timing almost exactly at rest but agreed only moderately on the intervals HRV is calculated from. No study cited here tested the Series 12 or Ultra 4.
Why this is rated Uncertain
Both anchor studies test current-generation Apple Watches against a medical reference, and neither cleanly affirms close HRV agreement. The Series 9 and Ultra 2 study found a consistent underestimate that failed its equivalence test, and the Series 6 study found near-perfect heart rate and beat timing alongside only moderate agreement for HRV intervals.
Two smaller studies point the other way for time-domain HRV at rest, including one that found the watch's HRV still tracked a mild stress response. With anchors that were studied but do not affirm, the rule rates the claim Uncertain: the heart rate signal is sound, and the HRV number built from it is less reliable.
- An anchor study was assessed but its own reported finding runs contrary to this claim, so the evidence is treated as unsettled rather than affirming.
- An anchor study was assessed but its own reported finding supports this claim only weakly, so the evidence is treated as unsettled rather than affirming.
Key results
- In 39 healthy adults providing 316 measurements over 14 days, Apple Watch Series 9 and Ultra 2 underestimated HRV by 8.31 ms on average against a Polar H10 with Kubios (MAPE 28.88 percent, mean absolute error 20.46 ms) and fell outside a 10 ms equivalence margin; resting heart rate differed by -0.08 bpm. (O'Grady et al., Sensors, 2024)
- In 78 healthy adults aged 20 to 75 measured against a Biopac 3-lead ECG, Apple Watch Series 6 R-R intervals had a mean absolute percentage error of 1.15 percent at rest, while N-N intervals had 31.31 percent, with agreement highest during rest. (Bonneval et al., Sensors, 2025)
- In 20 healthy volunteers compared with a Polar H7, Apple Watch RR intervals showed very good reliability and agreement (above 0.9); time-domain HRV was not significantly affected by gaps in the watch data, frequency-domain power was, and the watch's HRV still registered a mild mental stress response. (Hernando et al., Sensors, 2018)
What this finding covers
Applies to: HRV derived from Apple Watch Series 6, Series 9 and Ultra 2, and earlier models in one study, compared with ECG or a validated chest strap, mostly at rest in healthy adults.
Does not cover: Apple Watch Series 10 and later and Ultra 3 and 4, including the Series 12 and Ultra 4 with Apple's redesigned sensing system; HRV during exercise; people with arrhythmias; Apple's Readiness score.
Limitations
- The studies use different HRV measures and recording windows, so their error figures are not directly comparable.
- Apple Watch reports HRV as SDNN from short recordings, which is a different measure from the overnight RMSSD most rings and bands report.
- Sample sizes are modest, and two of the four studies are small.
