What Is a Good HRV for a 70 Year Old?

The reference values for the 70s, how small the groups behind them are, and how to read your own number.

Reference resting HRV (RMSSD) for men and women aged 70 to 79, how to read a wearable reading against it, and what the evidence says a high or low number means at this age.

For a woman aged 70 to 74, the median resting HRV is 18.3 ms, and 16.1 ms from 75. For a man it is 16.0 ms at 70 to 74 and 14.9 ms from 75. The study’s authors report no sex difference after 60, though the women’s medians in the table stay slightly higher. These medians come from 10-second resting ECGs, but the groups behind them are small, 403 women and 355 men aged 70 to 74, and the top of the normal range widens sharply, to 161.9 ms for men. At this age a high reading is not automatically good: in healthy adults the parasympathetic measure turns back upward after 60 in a pattern that may reflect age-related change. The most informative thing a device can show you is your own direction over weeks, read against the conditions of each night.

Small groups and a widening top end

The Lifelines reference had 8,643 women aged 45 to 49 and 403 aged 70 to 74, and only 172 women and 140 men aged 75 and over (European Journal of Preventive Cardiology, 2020). A median from a few hundred people is still a reasonable midpoint, but the percentile edges are much less stable.

Those edges also move in an unexpected direction. The 98th percentile for men is 86.6 ms at 60 to 64 and 161.9 ms at 70 to 74, and the women's median at 70 to 74 (18.3) is slightly above the 65 to 69 figure (17.8). A 24-hour ECG study of 543 healthy adults found RMSSD follows a U shape with a reversal above 60, which the authors suggest may reflect age-related autonomic dysfunction even in healthy people (Global Heart, 2020). An unusually high reading in your 70s is worth reading in context.

A systematic review of older-adult HRV reference values found 11 usable studies among 1,618 screened, with no standard method and widely varying values (Psychophysiology, 2024). The honest summary is that the 70s table is a rough guide.

What a low reading means at this age

Reading a night-time number in your 70s

In 47 adults aged 65 and over tracked across one summer, bedroom temperatures above 24 °C went with higher odds of a clinically relevant overnight HRV drop, rising to 2.9 times the odds at 28 to 32 °C (BMC Medicine, 2025). See bedroom temperature and HRV.

The device matters too. Fitbit and Pixel Watch compute RMSSD over the longest sleep (Google documentation), Oura shows a night-time average (Oura documentation), and Apple Watch reports SDNN, a different measure that cannot be read against this table (Apple HealthKit documentation). Against an overnight ECG, error ranged from 5.96 percent for Oura Gen 4 to 16.32 percent for Polar Grit X Pro (Physiological Reports, 2025). The 60s page and HRV by age cover the decades before.
