Does Fitness Still Matter Once You've Been Diagnosed With Cancer?
Cardiorespiratory fitness is usually discussed as something that prevents disease, but the mortality evidence doesn't stop once a diagnosis arrives.
This piece covers what mortality research shows about cardiorespiratory fitness and muscle strength in people already diagnosed with cancer, alongside longer-term fitness-and-cancer research in a cancer-free cohort. It does not cover fitness as cancer prevention in people without a diagnosis, or any specific exercise program.
Fitness content usually treats VO2 max as a number that lowers your risk of getting sick in the first place, which can make it feel irrelevant once a diagnosis has already happened. Research pooling 42 studies covering nearly 47,000 cancer patients found that higher cardiorespiratory fitness and muscle strength were tied to meaningfully lower all-cause mortality after diagnosis, including in advanced-stage cancers, showing the fitness-mortality link doesn't end where prevention ends.
A number people stop tracking once it stops being about prevention
Cardiorespiratory fitness gets talked about almost entirely as a before-the-fact number, something that lowers your odds of heart disease, of cancer, of an early death, if you keep it up over the years. Once someone actually receives a cancer diagnosis, the framing usually shifts entirely to treatment. Fitness quietly drops out of the conversation.
That shift assumes fitness stops being a meaningful signal once the disease it was supposed to help prevent has already arrived. The mortality research in diagnosed cancer patients doesn't support that assumption.

- Pooling 42 studies covering 46,694 cancer patients, those with high muscle strength or cardiorespiratory fitness had a 31 to 46% lower risk of all-cause mortality compared with those with low fitness levels, with each unit increase in cardiorespiratory fitness associated with an 18% reduction in cancer-specific mortality. The reduced-mortality association held in patients with advanced-stage cancers and specifically in lung and digestive cancers.

Before diagnosis and after diagnosis, same direction
These two lines of evidence are answering slightly different questions. The Copenhagen Male Study measured fitness in men who did not have cancer and tracked who eventually died from it decades later, over one of the longest follow-up periods in this evidence base. The Bettariga meta-analysis, by contrast, measured fitness and strength in people who already had a cancer diagnosis and tracked survival from there.
Put together, the two point the same direction rather than telling separate stories: fitness measured before disease predicts who later dies of cancer, and fitness measured after diagnosis predicts who survives it. Neither result depends on the other. But they line up in a way that undercuts the idea that fitness's relevance to cancer mortality is confined to prevention. What the meta-analysis doesn't establish
The meta-analysis reports an association between fitness or strength level and lower mortality risk after diagnosis. It pools 42 observational studies rather than randomizing patients to different fitness levels, so it can't rule out that healthier patients, those able to tolerate more intense treatment or with less advanced disease at diagnosis, are also the ones who happen to be fitter.
Nor does it identify a specific mechanism connecting fitness to survival in this population, only the statistical association itself, dichotomized as high versus low fitness or strength across a wide range of cancer types and stages.
The 46,694-patient meta-analysis is built from observational studies comparing high versus low fitness or strength, not from randomized trials assigning patients to different fitness levels, so it establishes an association with survival, not that raising fitness after diagnosis causes a person to live longer.
