Why this isn't as strange as it first sounds
Heart rate variability and resting heart rate already carry independent weight in cardiovascular risk research well beyond this one finding. A comparison across 692,217 adults in Asia and Europe found elevated resting heart rate predicted all-cause mortality on a scale comparable to hypertension itself, treating RHR as a risk factor in its own right rather than a stand-in for blood pressure.
That established track record is part of why a smartwatch's baseline HRV and activity readings showing up as predictors of future hypertension isn't a coincidental finding. It fits a broader pattern, autonomic and activity markers a watch already tracks are independently informative about cardiovascular risk, not proxies that only matter because they happen to correlate with blood pressure. Resting heart rate's own relationship to long-term health follows the same logic outside of hypertension specifically.

This is a prediction finding, not a screening tool. It shows that people who eventually developed hypertension already had different baseline patterns a year earlier, which is a genuinely different claim than saying a watch can flag an individual's current hypertension status the way a blood pressure cuff can.
