# Oura Ring HRV Accuracy Against ECG

Question assessedIs the Oura Ring's heart rate variability (HRV) reading accurate compared with an ECG?

KnowYourPrime evidence rating Established Strong, consistent evidence supports the claim.

Yes, for overnight HRV in healthy adults. In an independent test of five wearables against ECG over 536 nights, Oura Gen 4 and Gen 3 had the closest HRV agreement of any device; in 35 healthy adults at home, Oura showed low mean bias against a medical-grade ECG; and in 49 adults, Oura researchers found nightly HRV within 1.2 ms of ECG on average. Short 5-minute readings are noisier than night averages, and errors were larger in people aged 45 and older.

Finding KYP-ER-0024 Version 1.0 Reviewed October 24, 2026 Rating rule 1.6 [CC BY 4.0](https://creativecommons.org/licenses/by/4.0/)

## Why this is rated Established

Three anchor studies affirm close agreement with ECG for overnight HRV: one independent multi-device comparison, one independent home study, and one by researchers affiliated with Oura Health. With three clean affirming anchors and no contradicting study, the rule rates the claim Established.

A fourth anchor qualifies it: accurate HRV required strict data-quality filtering and averaging over at least 30 minutes, and errors were larger in older participants. The finding is about overnight readings in healthy adults, not readings taken during the day or in people with heart rhythm conditions.

How the rating rule reads this evidence

- Anchor-grade evidence affirms the core relationship.

## Key results

- Against a simultaneous ECG over 536 nights in 13 adults, nocturnal HRV concordance was 0.99 for Oura Gen 4 (MAPE 5.96 percent) and 0.97 for Oura Gen 3 (7.15 percent), against 0.94 for WHOOP 4.0, 0.87 for Garmin Fenix 6 and 0.82 for Polar Grit X Pro. (Dial et al., Physiological Reports, 2025)
- In 35 healthy adults monitored overnight at home against a Shimmer3 chest ECG, Oura heart rate and HRV showed low mean biases in both 5-minute and whole-night tests, with the dashboard RMSSD showing relatively low error variance. (Cao et al., Journal of Medical Internet Research, 2022)
- In 49 adults aged 15 to 72, nightly average HRV from the Oura Ring agreed with ECG at r squared 0.980 with a mean bias of -1.2 ms; the authors include researchers affiliated with Oura Health. (Kinnunen et al., Physiological Measurement, 2020)
- In 114 participants from two sleep-lab studies, accurate Oura HRV required at least 80 percent valid beats per 5-minute segment and averaging over 30 minutes or more, and more than half of participants aged 45 and older exceeded 10 percent median absolute error. (Liang et al., Sensors, 2024)
- Across 148 trials in five healthy adults, Oura resting rMSSD had a mean absolute percentage error of 6.84 percent (concordance 0.91) against multi-lead ECG. (Stone et al., Frontiers in Sports and Active Living, 2021)

## What this finding covers

**Applies to:** Overnight HRV (RMSSD and related measures) from the Oura Ring, Gen 3 and Gen 4 and earlier models in these studies, in healthy adults, compared with a simultaneous ECG.

**Does not cover:** Daytime or on-demand readings, HRV during exercise, people with arrhythmias, the Oura Readiness score built on HRV (see the readiness finding), and models released after these studies.

## Limitations

- The multi-device study that ranks Oura first had 13 participants, many nights each.
- One of the three affirming studies was carried out by researchers affiliated with Oura Health.
- Accuracy depends on how readings are filtered and averaged, which can change with firmware.
